Occupational Therapy Evaluation
Personalized evaluation to identify an individual’s strengths, challenges, and goals related to daily activities with a treatment plan developed to support function and quality of life. Treatment plan frequency is discussed with the individual and is a price per session fee after the evaluation.
A Home Safety Assessment can be included at a discounted rate when scheduled alongside the occupational therapy evaluation.
